It's not even just being denied coverage... several insurance companies say they'll pay for it, then you get billed. When I was looking at various inpatient facilities for myself I asked about my insurance at the time... apparently it was infamous for saying it would cover $500 a day (which sounds like a lot, but still leaves you paying $500 a day for 45+ days) then not even covering that.
